We are thrilled to present an opportunity for young creators to showcase their quilts at our upcoming show. This section outlines all the important information about entering a quilt as a youth participant.

Who Can Enter?

Anyone who is 17 and under on the first day of the show (June 5, 2026), is eligible to enter a quilt. It’s OK if the quilt was made with a little help. All sizes and styles are welcome; the only requirement is that your quilt must be finished, clean, and ready to hang.

All youth entrants will receive a participation ribbon, and the quilt will be proudly displayed at the event!

Sponsorship

To participate, the youth must be sponsored by a member of our quilt guild.
The sponsor helps the youth enter the show. The sponsor will submit the youth entry.
The sponsor is point of contact with the youth and their parent/guardian
….for the quilt entry and the parent/guardian consent.
The sponsor will communicate any updates or concerns.

Consent and Safety

Parent or guardian consent is required for each quilt that is entered.
If consent is not received by May 1st 2026, the quilt cannot be displayed.

Photography and Display

The youth quilt may be photographed for use on our website or in our newsletter.
On the Quilt Label and in any photographs, we will use only the youth’s first name and the initial of the last name and their age.
Photos of the youth will not be taken without parent/guardian consent.

Key Points About Entering a Quilt

The sponsor will enter the quilt on behalf of the youth.
The youth entry form must be submitted by April 15, 2026.
Parent/Guardian Consent must be submitted by May 1, 2026.

How to Enter

Paper Entry Form

The sponsor can use a paper entry form which includes both the youth quilt entry and the Parent/Guardian Consent. Print the PDF form in the link below.

The completed form can be mailed to the address on the bottom of the form or brought to a quilt guild meeting.

Or Online Entry Form – Two Steps

The Online Entry is a two-step process:

  • The sponsor submits the Youth Entry
  • The sponsor emails the Parent/Guardian so that they can complete the online consent form.
Step one ….Submit the Youth Entry

The sponsor uses the link below to enter the youth quilt into the show.

The sponsor will receive an email confirmation with the details of what has been entered.


Step two …. Email Parent/Guardian for Consent

Send the link below to the Parent/Guardian.

You can copy the link above and paste it into a personalized email for the parent/guardian. When the parent/guardian completes the consent; the guild will have an automatic record for their files.

When the youth entry is completed, the sponsor will receive a confirmation of the entry with the entry details. This email comes from ‘Forms Response Receipts <forms-receipts-noreply@google.com>’.

This confirmation email can be forwarded to the parent/guardian asking them to complete the consent form.  Consent needs to be obtained by May 1st, 2026.

Subject: Parental Consent Needed for Youth Quilt Entry – [Child’s Name]

Dear [Parent/Guardian Name], I am sponsoring [Child’s Name] for the ‘Muskoka Quilts’ from the Heart Quilt Show’ on June 5th and 6th, 2026.  The details of your child’s entry are in the email below.

We’re excited to include their quilt in the youth section.

To complete the entry, the guild requires parental or legal guardian consent by May 1st, 2026.

 This form confirms your permission for your child’s quilt to be in the quilt show and allows the guild to display and photograph the quilt during the show.
